I prefer the basic sheet done by Chris (solomoriah), but mostly because it is available in spreadsheet format where I have done a few tweaks.

I would probably like this Mike Hill one IF somebody got it into spreadsheet format (and preferring Open/LibreOffice format over excel).
Is it really the end, not some crazy dream?

Well, better late than never I guess. I apologize for the delay but I managed to come up with something here - https://docs.google.com/file/d/0BzCBl0E ... sp=sharing
I'd appreciate any feedback you have. If it's passable I can also make it fillable although it might be another few months before that's complete.
The problem with doing something really stupid to impress people is that they may just be impressed by how stupid you really are.

I apologize for how long this has taken but I've finally finished it. https://dl.dropboxusercontent.com/u/602 ... llable.pdf
Let me know if you find any errors/omissions/corrections, etc.

*NOTE: I use Foxit PDF Reader so I can't guarantee that it will look the same in other readers. You may need the Fritz Quadrata font as well.
