This is my current choice for two-weapon fighting for my games;

A player (or monster) wielding a one handed melee weapon in each hand is allowed to attack with the offhand weapon if they miss with their primary weapon. Damage from the offhand attack is scored normally based on the offhand weapon.

Which is very similar to MedievalMan's, except I don't penalize the 2nd attack (if/when it occurs).

When it comes to two-weapon fighting... no one way really seems right. I am always flip-flopping on this one.
Is it really the end, not some crazy dream?

I just think however you handle it, it should feel like a two weapon attack, not just a better one weapon attack.

Well, considering a trained fighter's weak side will still be weaker than his strong side, there should at least be a -1 penalty to the damage (possibly even more) given that the weak side is... well... weaker.

I agree with that, but I see the requirement that the off hand weapon be a smaller weapon (less damage) than the prime hand takes care of the off hand being weaker. Give the penalty to the attack, but let a hit be a hit and the damage be normal for the weapon in use.

The mechanic I use was invented by Gary; that makes it good enough for me.

Thats the one I would like if I was playing at higher levels and I wanted to wade through some pesky goblins. At first level its really not very useful unless you like whiffing a lot. :)

If that was true you would be playing AD&D 1st ed and not BFRPG.
