I have done a new cover layout with some of the missing stuff added back in (code and descriptive string) with the gray band and the title font Rocketfuel that you requested. LA1 is the "normal" code since I usually use initials, but if you have another preference I'm agreeable.

I'm building an R21 right now. Anyone proofing R20, just post your corrections here as noted above.
Leon, I've integrated the four images you tagged to replace the gray boxes. If you want to fill in white spaces, feel free to post a list here for me. Also, I changed the title page to the Rocketfuel font to match the cover.

Basically no textual difference between R20 and R21, but the page numbers might be off a bit. You don't need to post corrections, necessarily, but give me some context to search for to find the errors.
