Rolling both attack and damage die together allows to describe more "accurately" the attack quality even if it misses.

If you can't see your opponent, but can somehow detect his or her location in a general sense, you have a -4 (just as if the opponent were invisible). You can't hit your opponent at all if you can't somehow figure out where said opponent is.

If your unseen opponent is aware of you and his movement isn't obstructed I would agree with that, but if he isn't actively trying to dodge your attack or has somehow been cornered I can see a lucky hit making contact if the attacker is just swinging away randomly.

Thanks a lot for the help everyone, it is really useful, and I love the game. It was used to introduce 4 people to real pen and paper role playing including myself.
