I was thinking of the type of monster that makes things look great and gets their victims to willingly go, but those who resist (or save) see the water churn to red when they slaughter the ones who went.

Maybe not a monster, but an enchanted pond...

Pond of Narcissism (or other name)
This pond shines with clear crystal water, anyone seen their reflection on it must save vs spell or be charmed by their own reflection, such character will spend its time looking at the reflection and will refuse to engage in other activities. The target receives a new saving throw each hour if it has an Intelligence of 13 or greater, every 2d4 hours if its Intelligence is 9-12, or every day if its Intelligence is 8 or less.
Finally, a character rolling a natural 1 on any of its saving throws will be so delighted by himself that will dive into the pond in order to be with himself even at the cost of his life. The pond will absorb any dead creature in a matter of hours and any object in a couple of days.

It could be that the font in 11 is needed to purify the 4 items before they can be used.

The demonic door could be inert. Or it could be animated and taunting the players. It could be a construct (more a golem type than a living statue) difficult to damage. It could be its hand that is poking out in room 16. Maybe it is animated by the soul of the dead jester of the king or maybe it's something anchored in the neither planes. It could offer hints to the players but would spit out terrifying creatures after each one. Would do even worse if attacked directly.

Maybe the head and the hand were part of an Ozymandias-type giant statue, animated of course, that was somehow broken up and buried here.

"Tomb" or "Catacombs" implies undead as a major enemy; "Dungeon" implies very little, by comparison, but adds little flavor. Neither "Shrine" nor "Temple" seem appropriate here.

Of course, rather than the location, the title could be about the boss villain. Or both ("Tomb of Bigbadguy" for example).

What about "Ruins" of something or other. "Ruins of the Gate of Varos" or something like that - Varos being the random name that popped into my head for the BBEG

Varos works for me, and so does the title. Is everyone else in agreement?

I've uploaded a starter manuscript (see the first post) with everyone's contributions. At least, I think I got everyone's stuff in there. Naturally it's disorganized, but that will come in time.

WHAT WE NEED NOW:

1. Everyone is listed in the contributor's list by handle. I should know some of your credits, but I have forgotten. Please PM me if you want something else besides your forum handle in there.

2. Read over the collected materials, and comment on any room you want. Assume nothing is written in stone (so to speak); we want to see ideas. This is a beautiful dungeon, and it deserves our best work. This naturally means everyone needs to have a thick skin about comments that contradict your ideas.

3. Francisco, please illuminate me (us, even): What is the diagram in the upper right of the map, the one with the arrow, telling me?
