Thanks once again for the excellent editing.

Regarding #2, yes all of the ghouls come from area 11.

As far as the HP boxes on the random encounter tables go, what is the correct format for a variable number of creatures? Should I randomly generate the maximal number HP lines (e.g. 8 lines for 2d4 creatures or 6 lines if 1d6) or a single line that can be used for each?

For HP boxes you can decide on the number of creatures and then use the HD to determine the number of boxes. You can even have 15 creatures and in the Name of the stat block put 1d6 this would allow and option of 3 to 15 encounters (the first is if all the rolls are high and the last is if you, by chance, roll 15 ones in a row (personally I'd have the die inspected if that would happen :) ).

I updated the doc on the first post to r6, which includes HP check boxes for wandering monsters that didn't have them, and clarifies the origin area of the ghouls.

Here is what I did to get to r8:

Made sure that last line of HP blocks is set to HPBlockEnd Style.

Page 8 – 4. 1d4+1 Assassin Vines – HP Blocks
The one with 32 only had 20 boxes. Added another line of 12 boxes.

Page 8 – 6. 10d4 Pixies – HP Blocks
Put into two columns.

Page 9 – 8. 2d6 Brush Horrors – HP Blocks
Put into two columns.

Put r8 on first post.
