yet the table contains two lethal options in it.

That should read: "Unless the random location is a lethal plane by its nature (Plane of Fire, etc), the location the party is teleported to will not immediately lethal; i.e. the party won't appear 100' up in the air above a jagged cliff. Teleporting on the surface of the ocean, might, of course, become deadly almost immediately for heavily armored party members."
