I really liked Maptool for 'At The Table' virtual mapping as well. Instead of using a roll out map I used a monitor hooked to my laptop. I could run both a players and DM Map so I could take advantage of Fog of War and Lighting.

The short answer is, it's up to the GM to decide how to interpret it. As to the image in the book... when you run a collaborative project and accept submissions from many people, sometimes there is a variation in each person's vision.
