Encounter distance is critical factor. I can't see anything in Core Rules determining this (is there anything?). I always say you can move diagonally at the same cost. It's a goose and gander thing. If you can do it, the baddies can as well.

Then again, I often use 5' squares (ducks for cover) so maybe I might rethink with 10 footers.

If you are going to roll, do it beforehand. If you are doing it on the fly, assign standard values (5 for an 8-sided die, for example).

In my experience, the thing you want to avoid is setting up the ogre as the BBEG and then rolling the lamest hp ever.
