That is a good catch. I would make it a tad more explicit as "if a spellcaster is successfully attacked or otherwise struck in combat (including magical effects) on the Initiative number on which he or she is casting a spell, the spell is spoiled and lost." I like the word "interrupt...

I like it. I purposely didn't say Dexterity "bonus" since there might be a Dex penalty. Maybe instead, "Dexterity modifier"?

For placement, the next logical place (IMO) would be as you suggest on/near page 11 right before the Armor and Shields Table.
