I guess you are using Luigi Castellani's barbarian. They would have the chance of higher HP but they can also roll low values. I do add the con modifier, be it positive or negative to the HD roll for recovering health.
If I where using a flat value I would also add it, and in the case of a 0 or lower value the pc would heal 1HP after some days.
